You may be finding yourself in the fortunate position to be applying for citizenship or changing your status to permanent resident. There definitely is a lot to do and one of those things may be to have a medical examination. This is where we can help you check one thing off your “to-do” list. Dr. Mignon is our resident civil surgeon at Health & Wellness Center of North Florida and looks forward to seeing you through this part of the process. We provide this service to a wide area including Tallahassee, and the Florida panhandle.
First of all, it’s important to know that it is a requirement for the medical examination to be performed by a civil surgeon. A civil surgeon is a doctor who has been chosen by the United States Citizenship and Immigrations Service (USCIS) to perform medical examinations for people seeking an adjustment of status to permanent residence in the United States.
The reason for the medical exam is to confirm that you are in good health. In addition to the physical examination we will do the following:
- Review of medical history: You will supply information about your health in regards to things such as previous illnesses, disabilities, and chest X-rays. You also need to bring vaccination records with you.
- Tuberculin (TB) skin test: Anyone two year of age and older who is applying is required to have this skin test. In some cases, if the applicant is under two years of age, he or she may also have to have one. After the skin test, it will be necessary for you to come back within 48 to 72 hours so the results can be read.
- Blood test (Serologic): Those who are fifteen years old and older will have a blood test for HIV and syphilis.
- Vaccinations: There are several vaccinations that are required. These include but may not be limited to: rubella, measles, mumps, polio, tetanus, diphtheria toxoids, pertussis, influenza type B, and hepatitis B. The ones you receive depend on what your vaccination records reveal.
The Form I-693 is used to report the results of the medical exam. You will fill out the first part and/or we can assist you with this. When the exam is completed, the rest of the form will be filled out by Dr. Mignon, placed in an envelope, and returned to you.
As a reminder you will need to bring your passport or other form of photo identification issued by the government. Dr. Mignon is experienced and knowledgeable with this medical examination process.
